Community Health Centers are local, non-profit, health care organizations in communities across the nation. For nearly 40 years, the national network of health centers has provided high quality affordable primary care. Also known as Federally Qualified Health Centers, they are located in areas where care is needed but scarce.

How do I become a Four Corners Primary Care Centers (FCPC) patient?
You may make an appointment at (770) 806-2928 or come in on a walk-in basis. We accept walk-in patients on a first come, first serve basis. Please bring with you valid photo ID (It does not have to be a GA ID) and proof of insurance or income if uninsured.
Do I need to make an appointment?
We encourage you to make an appointment to keep your wait time as short as possible. However, we will accept walk-in patients on a first come, first serve basis.
What do I need to bring to my appointment?
- State issued ID/Driver’s license
- Passport or Consulate ID
- If you have any type of medical insurance, bring insurance card.
- If you do not have medical Insurance please bring last three pay stubs or most recent bank statements.
- If self-employed bring most recent Federal Tax Return.
Do you have a bi-lingual staff?
Yes. From the Front Desk staff to Providers, we have a number of people available who are multi-lingual and will be able to help you. Languages spoken include English, Spanish, French and Portuguese.
Are you open at night or on weekends?
Four Corners Primary Care Centers is open Monday – Friday 7:00 a.m. -5:00 p.m., with the exception of state holidays. To better accommodate to our Pediatric Patients Four Corners Primary Care Centers Norcross Pediatrics department is open for from 7:00 a.m. to 5:00 p.m. during the summer.
Do I need a green card to be seen?
No. Four Corners Primary Care Center sees patients without regard to their immigration status.
Are services free?
No, services at Four Corners Primary Care Centers are not free. Services are offered on a sliding fee scale based on income and household size.
Will Four Corners Primary Care Centers bill me for services, or do I have to pay when I get there?
It is Four Corners Primary Care Centers policy to expect payment at the time services are rendered.
Can I come to Four Corners Primary Care Centers (FCPC) if I have insurance?
Yes. We accept a large number of health insurance plans including Medicaid and Medicare, and Plans under The Affordable Care Act. Be sure to bring your insurance card with you for any visits at FCPC.

My child is starting school; can Four Corners Primary Care Centers provide her with the right Immunizations, Hearing and Vision Screening, and a Physical?
Yes! Four Corners Primary Care Center provides physical exams to people of all ages, including sports physicals. We provide immunizations for children of all ages, adults, as well as seasonal flu shots, and School required Hearing and Vision Screenings.
My spouse has a chronic medical condition that requires monitoring by a physician, is this beyond the scope of care provided at Four Corners Primary Care Centers?
No. At Four Corners Primary Care Centers, we provide care to many patients with diabetes, Cholesterol, hypertension, heart disease, and other chronic illnesses.
